Missed Appointments and late cancellations inconvenience those individuals who need access to medical care in a timely manner. We would like to remind you of our office policy regarding missed appointments. This policy enables us to better utilize available appointments for our patients in need of medical care.

Cancellation of an appointment
In order to be respectful of the medical needs of other patients, please be courteous and call Acacia Health Partners promptly if you are unable to show up for an appointment. This time will be reallocated to someone who is in need of treatment. If it is necessary for you to cancel your appointment, we require that you call at least 24 hours in advance. Appointments are in high demand, and your early notification will give another person the possibility to have access to care.
First late cancellation/Missed appointment

Second late cancellation/Missed appointment
$120 fee will be applied to your account
Third late Cancellation/Missed appointment:
$120 fee will be applied to your account and it will result in a discharge from the practice.
Late policy
If you are running late to your appointment, please contact the office by phone to notify us as soon as possible. In our efforts to provide quality care to you and other patients being seen, as well as to minimize your wait time, our office has implemented a late arrival policy. If you will be more than 10 minutes late to the appointment, please be advised you may be asked to reschedule.
STD/LTD/Disability Paperwork
Medical documents/forms (including FMLA, STD, ESA) will be considered only for patients established for at least three months. A fee will be applied of $20 per page.
